Step 1: Determine if your area is eligble.

Find out if you live in an eligible county and have transportation needs related to healthcare, food access, pharmacy services, employment, education, or other approved essential services.

Step 2: Contact your Mobility Navigator

You MUST contact your Mobility Navigator before taking a trip.

The Mobility Navigator will

1. Confirm eligibility
2. Discuss transportation options
3. Provide a voucher or authorization number if trip is eligible
4. Provide information on how trip will be reimbursed and what documentation is required
5. Determine whether another transportation option is available

IMPORTANT: Your Mobility Navigator must be contacted prior to each trip to ensure reimbursement.

Step 3: Travel to the approved destination using their own vehicle or a volunteer driver, family member, friend, or other transportation arrangement.

Step 4: Keep required documentation, including date of trip, destination, purpose of trip, miles traveled, voucher/authorization number, required receipts, and supporting documentation.

Step 5: Submit for reimbursement. Submit the trip log and required documentation to the Mobility Navigator for review and reimbursement. The documents submitted must be legible and signatures must be provided on the mileage reimbursement form transportation arrangement.
