SECTION 5310 – COORDINATED HUMAN SERVICES TRANSPORTATION PLAN

Dear Community Stakeholder,

As you may know, the Oklahoma Department of Transportation (ODOT) is completing a Coordinated Human Services Transportation Plan to identify transportation services, gaps and unmet needs for seniors and individuals with disabilities. Once complete, this transportation plan will provide recommendations to better serve these communities across the state, as ODOT assumes responsibility of administering the Section 5310 program previously administered by the Oklahoma Department of Human Services.

As an identified stakeholder, your input is needed to help identify transportation services, gaps and unmet needs. In addition to taking the survey on behalf of your agency/organization, we request you assist ODOT in promoting the survey to your clients and networks – specifically providing the opportunity to Oklahoma community seniors and individuals with disabilities.

Please complete the below survey and share it with your networks by Tuesday, July 13, 2020. Additional engagement opportunities in the form of Telephone Town Halls will be scheduled across the state in the coming weeks. We will share these dates and details as soon as they are finalized.
